Insurance: When a claim is submitted to a payer, the applicable payer contract and allowed amount control. This private-pay schedule does not replace contracted reimbursement terms.

Reduced fees: Any reduced fee must be authorized through the ARBH Sliding Fee Discount Schedule or another written, consistently applied policy.

Non-clinical services: Coaching, mentorship, educational, consultation, and most workshop services are not psychotherapy and should not be represented as insurance-reimbursable clinical treatment.

Annual review: Leadership should review this schedule at least annually and update Qualifacts only after the revised schedule is approved.
